Player Documents
At each game, each team brings with them a binder which validates that the kids are who they claim to be and are eligible to play. The binder will contain the Player Contract, Pop Warner Physical Form, copy of Birth Certificate, copy of Grades and proof of residency.
Prior to the first practice, players must have all of the following turned in to their team.
- Signed Pop Warner Players Contract (see link below)
- Pop Warner Physical Form signed by doctor. Only a signed Pop Warner form is valid. This is a national federation rule. (see link below)
Prior to the certification of the team binders, players must have all of the following turned in to their team.
The league will certify binders in the practice prior to the Official Weigh In, the Saturday prior to our first game.
- COPY of Grades from Q4/Final Semester from previous school year (please keep your originals)
- COPY of Birth Certificate (absolutely no originals!)
**We will NOT return any documents. All documents are shredded at the end of season.**
